I fully support and approve my child’s participation in the Barbara Ingram Summer Arts Academy. I agree to comply and
abide by all Academy and Camp rules and regulations.
I further agree that the Summer Arts Academy and the Barbara Ingram School for the Arts Foundation, Inc. has my
permission to take photographic images and audio and video recording of my child during the camp. I further consent to
and authorize the Summer Arts Academy and the Barbara Ingram School for the Arts Foundation, Inc. to use and
reproduce any such photographic images and audio and video recordings and to circulate and publicize the same by all
means, including but not limited to, newspaper and other print media, television media, brochures, pamphlets,
marketing materials, and websites for future advertising or the Academy and/or the Foundation.
I also assume and accept full financial responsibility for any and all health care cost included but not limited to, the cost
of medication, first aid, x-rays, lab work, hospitalization, and other medical care or service that arises during the camp.

Refund Information

You may cancel your application for the Summer Arts Academy and receive a refund (less $25.00 non-refundable administrative processing fee) only if the request for cancellation is made at least four (4) weeks prior to the opening of that camp session. No money will be refunded for any reason after the above-referenced four (4) week deadline. Students arriving late for camp or leaving early will not be eligible for monetary compensation or refund. If you are placed on a “waiting list” and do not attend the Summer Arts Academy, your deposit will be returned.
